Each individual and family only have to take the eligibility assessment once using the form below. Multiple entries received may delay the process to assist you with rental and utility assistance.
We have paused our eligibility assessment as of Saturday, January 16, at 12:30 pm due to the high volume of eligibility assessments received.
Our Housing Specialists are currently processing the rental and utility assistance requests received and appreciate your patience as they work hard to make the eligibility assessment form available very soon.
